Sentencing, treatment can be seen as retribution or punishment. Judges express the need to have better clinical guidance in order to shape the appropriate and specific treatment interventions. Assessment of addiction is limited and impacts treatment planning and reentry choices: There is a need for better screening and assessment strategies while inmates are incarcerated. Because addiction assessments are limited and not comprehensive, treatment planning during incarceration and the re-entry process has limited information to address the problems of addiction. The discharge planning forms do not have enough substance abuse questions and information to prepare the inmate to manage cues and triggers. The level of ASI severity amongst inmates should be the cue to do more comprehensive addiction assessments. In addition, NJ data on inmates only describes addiction severity as measured by the ASI not diagnoses. This maintains a lack of clarity on the actual number of substance abusers within the NJ prisons and jails. More accurate information is needed on the rates of substance use disorders amongst inmates and the severity of related problems, including co-occurring mental illness and medical illness. The lack of diagnostic clarity may in fact be one of the reasons that addiction treatment and recovery services are not universally provided throughout the legal system. Most evidence-based addiction treatments are not provided in the correctional system: Evidence-based addiction psychosocial and medication treatments are not used in the correctional Therapeutic Communities, therefore inmates are not receiving the standard of care that community based addiction treatment programs provide. This limitation is a missed opportunity for inmates to be better prepared for reentry. Psychosocial treatments help enhance motivation, understand triggers to use and how to manage them, and increased understanding of how to manage stress, anxiety, depression, and interpersonal difficulties. Addiction treatment medications methadone, naltrexone, antabuse, etc ; are another evidence-based clinical tool that can ensure abstinence through pharmacology creating a "drug free environment" even within the community setting. Psychiatric medications are appropriate for inmates with co-occurring addiction and mental illness and are also underutilized. Obstacles to integrating these approaches include limited resources, few staff trained in these modalities, the lack of medical staff working in the TCs, the division in corrections between addiction and mental health services, and the historical anti-medication roots of the Therapeutic Community model. Mental health treatment services are separate from substance abuse treatment services: The disconnect between mental health and addiction treatment services obviously inhibits appropriate co-occurring mental illness and addiction disorder treatment while incarcerated, and this disconnect also impacts reentry referrals and treatment planning. In addition, because of limited resources only the mental health system has the medical services needed to provide specific medication treatments. This inhibits inmates in the correctional TCs to have less severe mental illnesses addressed appropriately, including mild to moderate depression, post-traumatic stress disorder, learning disabilities, cognitive impairment, and personality disorders.

Experiment 1b of the Project to Intercompare Regional Climate Simulations PIRCS ; is a 60 day simulation of the 1993 flood over the upper Mississippi River basin. In this study, the investigators identified 23 patients who developed West Nile virus infection within 4 weeks of a transfusion of leukoreduced and nonreduced erythrocytes, platelets, or fresh frozen plasma. They linked 16 donors with evidence of viremia at donation to the 23 infected recipients. Of these donors, 9 reported viral symptoms before or after donation, 5 were asymptomatic, and 2 were lost to follow-up. At donation, all 16 blood donors were negative for West Nile virusspecific IgM antibody and were often near the limits of sensitivity of nucleic acid assays. Forty-three percent of patients who received transfusions n 10 ; were immunocompromised, had recently undergone transplantation, or had cancer. In addition, 35% n 8 ; were at least 70 years of age and had significant illnesses. Illness in patients who received transfusions began 2 to 21 days median, 10 days ; after the transfusion; immunocompromised recipients tended to have longer incubation periods. All of the transfusion recipients from these donors developed antibodies, showing that transmission was highly efficient. This study proved that transfused erythrocytes, platelets, and fresh frozen plasma could transmit West Nile virus. Beginning in June 2003, transfusion facilities began using a nucleic acid assay to screen all U.S. blood for West Nile virus. During the next 8 weeks, blood banks screened 1 million donations and removed 163 positive donations from the blood supply. Screening is particularly important because research has shown that 80% of West Nile virus infected persons are asymptomatic and have viremia that lasts about a week. West Nile virus is not a passing phenomenon. Immunosuppressed patients receiving contaminated organs or blood products appear to be at increased risk for severe disease, and the incubation period and period of viremia may be prolonged. The virus has also been transmitted through breastfeeding, after physical trauma in the laboratory, and vertically from mother to child and eletriptan.

The tactile system, like its visual and auditory counterparts, may be comprised of at least two parallel functional pathways one for processing identity and another for spatial attributes `what' and `where' pathways, respectively ; . To date, these pathways have only been investigated with hemodynamic imaging methods and with complex stimuli. Here, electrical neuroimaging investigated the relative time course of differential activity along these pathways in response to identical physical stimulation. There were 2 types of blocks of trials. `What' blocks varied stimuli in their frequency 22.5Hz vs. 110Hz ; independently of their location left vs. right hand ; , and subjects responded to a target frequency that occurred 10% of the time. `Where' blocks, varied the same stimuli in their location independently of their frequency, with subjects responded to a target location. Continuous white noise masked any sounds. Each participant completed 8 blocks of trials, fully counterbalancing the design and eliminating any physical differences between `what' and `where' responses. Only non-target trials were analyzed, and responses to each hand were separately analyzed. Within the initial 150ms, electrophysiological responses to `what' and `where' conditions significantly differed.
