Significant correlation between E and F r 0.063; P 0.0921 ; . Circadian variations Figs. 1 and 2, Table 3 ; in the serum E concentrations among our healthy subjects were seen to parallel those classically described for F; however, their individual E concentrations were practically identical at 0800 and noon. Circadian variations were also found in their saliva and urine. The highest E concentrations were observed in these subjects' serum and saliva at 0800 and the lowest at midnight. In urine this occured at noon and at 0400. The F E ratio fluctuated significantly over the course of the day Fig. 2 and Table 3 ; ANOVA P 0.001 for serum and saliva, P 0.045 for urine ; . In serum and saliva, the highest F E ratio was found at 0800 and in urine at 1600 P 0.001 ; . This ratio was significantly lower for urine during the day 0800 1600 ; than at night 2000 0400 ; P 0.0042 ; . Dynamic tests Table 4 ; . Synacthen stimulation tests: While a quite classically significant increase in the serum F concentration was observed in our healthy subjects 1 h after injection of short-acting Synacthen, a significant decrease in E was found. In contrast, serum E was found to be increased 4 h after administration of long-acting Synacthen. However, both these stimulation tests resulted in a significant increase in the F E ratio P 0.001 ; . DXM suppression test: Serum F and E significantly decreased P 0.001 ; in our healthy subjects during the DXM suppression test. However, the decrease in F was greater than that of E, resulting in a nonsignificant decrease in the F E ratio.

After treatment for acute Lyme disease, some patients develop chronic symptoms such as musculoskeletal pain, neurocognitive symptoms, dysesthesia, and fatigue. Klempner and colleagues asked whether prolonged antimicrobial therapy helps these patients. Their placebo-controlled trial of intravenous ceftriaxone, 2 g d for 1 month, followed by oral doxycycline, 200 mg d for 2 months, found that prolonged treatment provided no further benefit. As a result, the study was discontinued. Clearly, it is unwise to expose the small minority of patients who have chronic sequelae of Lyme disease to the potential health risks and costs of longterm antimicrobial therapy.
Introduction For many years the coast between Sacca di Goro and the mouth of the Reno River has been extensively studied because of the presence of loggerhead turtles Caretta caretta Vallini, 2000, Vallini et al., 2000; 2001 ; . Through a research project carried out until 2002 with CHELON's Marine Turtle Conservation and Research Program, attention was focused on the study of the interaction between the fishing methods most frequently used in these waters bottom trawls and midwater pair trawls ; and the loggerhead turtles. The analysis was performed on healthy live animals, on animals with any kind of problems preventing their release into the sea hosted by the Fondazione Ceteacea di Riccione ; , and stranded animals, both dead and alive. Since 2000 the Ferrara section of Istituto Zooprofilalttico Sperimentale of Lombardia and Emilia Romagna has offered its support to the protection of marine turtles that frequent the waters of Lidi Ferraresi. In 2002 IZSLER proposed and obtained the approval from the Ministery of Health and "Politiche Agricole e Forestali", as a center for receiving and assisting turtles injured or in difficulty, and providing shelter in those cases where objective or logistical problems did not allow the sheltering in specialized centers or for uncomplicated cases requiring a subsequent short-term housing. Materials and Methods Anamnesis - On July 13th, 2002 the Coast Guard harbour office of Porto Garibaldi got a report of a turtle in difficulty, close to the breakwater cliffs, by guests of a camping resort. The large turtle was taken to the beach and appeared to be in severe shock, but did react to eye reflex. We arrived in less than 30 minutes during which, by phone, we indicated some emergency actions to be taken in order to limit the stress to the animal. Clinical Signs The clinical examination indicated debilitation, marked malnutrition, dehydratation, and a weakly sensory state. Moreover, we observed the absence of cloacal reflex, but uninterrupted presence of eye reflex ; , an anemical mucosa, and respiratory difficulty. Three big continuity breaks were detected on the carapace, due to some traumatic event, probably propeller damage. The central break Fig.1 ; was of concern due to its seriousness and size 19, 2 cm x 2.5 cm ; , from the XII right marginal to the V neural. The animal also presented a ventral dermaskeletal fracture at two different points Fig.2 ; . Under closer examination the wounds were deep and necrotic, and covered with various epibionts and barnacles including Lepas anatifera, Chelonibia testudinaria, Platylepas hexastilus, Caprella acanthifera, Nereis sp., Ozobranchus sp., Serpula sp., and Mytilus galloprovincialis. With the locomotor apparatus we found an anatomo-functional alteration of the right anterior and posterior flippers. Because of the serious debilitated state and the big size of the animal Male, CCL max: 83.5 cm; CCW 76.4 cm, weight 58.9 Kg ; we preferred an intervention in place until the next morning in order to avoid movement during the night. Anti-shock therapy was immediately administered with the administration of 5% glucose solution 1 lt ; and dexamethasone 0, 2 mg kg ; subcuteneously. In addition, an antibiotic coverage was provided by administering a wide-spectrum antibiotic by phleboclysis 200 mg ciprofloxcin total ; . An accurate cleaning and a first curettage of the wounds were also performed. The turtle was then moved to the veterinary clinic. From July 15 the subject was maintained in an external brickpool cm 167 x 148 x 40 ; at IZSLER. In the beginning the water level was kept low, with 494 liters of water, in order to favor respiration and wound healing. Subsequently, the water level was increased to 676 lt. From September 2002, the turtle was housed indoors, in a pool made of fiberglass 165 x165 x 102 ; containing a mechanical and biological filter 240 lt. ; , and 800 liters of artificial salt water.

Where F and F0 are the fluorescence intensity of BSA in the presence and absence of CPB, respectively, and [Q] is the concentration of CPB. The obtained stacking efficiencies were listed in Table 1. From this table, it can be seen that Ddobsd for H-2 and H-3 are 0.179 and 0.085, and the corresponding efficiency are 0.76 and 0.73, respectively. This indicates that these two protons are the closest to the aromatic rings of BSA. When CPB reached its second CMC, these signals become as narrow as those of the pure CPB except a little upfield moving, indicating that the headgroups of CPB gain their. History of premature CAD in a first-degree relative parents or siblings ; has been found to be the single best predictor of risk even when patients with an inherited dyslipidemia are excluded. Therefore, a family history of CAD should be documented early in childhood. Because parents of infants and young children may be too young to have CAD themselves, a positive family history is defined as documented myocardial infarction, angiographic documentation of CAD, angina pectoris, or sudden cardiac death in first- or second-degree relatives parents, siblings, grandparents, or blood-related aunts and uncles ; 55 years of age. If a positive family history is identified, information about CVD risk and about strategies to reduce risk should be introduced. Parents with significant CVD risk factors need to be referred for evaluation if this has not been done previously. Because the health status of family members is still evolving in young families, family history must be updated annually. If the family history becomes positive for CAD at any time, a review of CVD risk and refer.

A nation-wide comparison between outcomes of term infants presenting in breech or vertex born vaginally versus planned caesarean section Ulander V-M, Gissler M, Nuutila M and Ylikorkala O Department of Obstetrics and Gynecology, Helsinki University Central Hospital, Helsinki, Finland veli-matti.ulander hus.fi Objective: Comparisons between term breech infants born vaginally or by planned caesarean section CS ; have neglected the fact that vertex vaginal delivery is also associated with neonatal morbidity. This may lead to unrealistically high health expectations of infants born vaginally. Patients and Methods: We compared the outcome of infants born by breech vaginal n 1 270 ; or by vertex vaginal delivery n 128 683 ; or through planned caesarean section CS ; in breech n 1 640 ; or vertex n 4 997 ; . The study population was collected from 139 068 non-malformed singleton term infants born after uncomplicated pregnancy between 37 and 42 weeks of gestation and entered into the Finnish Medical Birth Register MBR ; from 1987 to 1989. The MBR is linked to a number of other nation-wide registers such as the birth death-certificate-register and hospital discharge-register, and this enables a large follow-up study. Results: One infant perinatal death occurred in the breech vaginal group 1 1270, 0.08% ; and 23 deaths in the vertex vaginal group 23 128 683, ; P 0.112, NS ; , but none in either CS group. Breech vaginal delivery was associated with increased risk of Apgar score 6 at age 1 min OR 7.65, CI 6.41-9.12 ; or at age 5 min OR 6.42, CI 4.36-9.45 ; as compared to vertex vaginal delivery. These odd rations were also elevated OR 4.59, CI 3.48-7.04 and OR 7.58, CI 3.09-18.66, respectively ; when compared to breech planned CS. Yet the risk for birth trauma of infants in the breech vaginal group was smaller OR 0.70, CI 0.51-0.96 ; than that in the vertex vaginal group. No difference existed in the incidence of neonatal intracranial haemorrhage. A number of other neonatal complications convulsion, respiratory distress, gastrointestinal disease ; occurred equally commonly in each group. We followed the infants to the age of 7 years. Breech infants born vaginally needed fewer admissions OR 0.58, CI 0.47-0.72 ; to outpatient departments than did breech infants born by planned CS. The cumulative incidence of long-term morbidity in the breech vaginal group was smaller OR 0.47, CI 0.28-0.80 ; than that in the breech planned CS. We also studied the maturity for starting school and school performance during the first two school years in one subgroup of children, and these variables showed no dependence on mode of delivery. Conclusions: Thus, apart from Apgar suppression, elective vaginal delivery of a full-term breech fetus seems not to lead to long-term morbidity. There is therefore, scarcely any need to change the guidelines for the management of full-term breech presentation, at least in Finland or in other countries providing prenatal care similar to that of Finland and megace.
